Eligible employees are entitled to Long-Term Disability (LTD) benefits if they have been medically certified as disabled from performing the duties of their regular occupation due to a non-work-related illness or injury.

To apply for LTD benefits related to a non-industrial injury or illness, submit the following forms to the LTD Office:

• Attending Physician Statement Note: If your health plan carrier is Kaiser, please contact the Kaiser Disability Office to request an Attending Physician Statement.

In order to return to work, a physician must release you to either full duty or light duty with restrictions. If your physician has released you to return to duty, please submit the appropriate form below to the LTD Office.
