Industrial Injury or Illness Claims
Eligible employees are entitled to Long-Term Disability (LTD) benefits if they have been medically certified as disabled from all gainful employment due to an illness or injury that is work-related.
Important highlights of this benefit include, but are not limited to:
- Eligible employees may be entitled to LTD benefits for an Industrial Disability where they do not qualify for Industrial Leave benefits and are declared disabled from their regular occupation.
- Eligible employees may be entitled to LTD benefits for an Industrial Disability for a period of up to 12 months after Industrial Leave or 4850 benefits have been exhausted for such disability.
- Eligible Employee must be deemed totally disabled from all gainful employment, to be considered for the benefit after one year of 4850 or Industrial Leave benefits are paid. If an Employee can return to work in any capacity, the LTD plan will not provide a benefit.
- Flexible Benefits Plan (FBP) Credits will be allotted in the amount of the Employee Only cost of Medical and Basic Life Insurance combined, up to their standard FBP Credit.
To apply for LTD benefits related to an industrial injury or illness, submit the following forms to the LTD Office:

- Attending Physician Statement Note: If your health plan carrier is Kaiser, please contact the Kaiser Disability Office to request an Attending Physician Statement.

In order to return to work, a physician must release you to either full duty or light duty with restrictions. If your physician has released you to return to duty, please submit the appropriate form below to the LTD Office.
